I'm a pediatrician. I would use this book maybe 4 times a year when a constellation of symptoms and signs has me stumped... most commonly at 2am when there are fewer colleagues around to bounce off. I am writing this at just such a time, as I was using the book and wanted to know if a more upto date edition had been published. This says two things: first, that I value the book highly enough that I would buy it again; second, that some of the statement of "facts" are now way out of date. It is an ambitious book that takes a symptom or sign (eg vomiting) and lists comprehensively the potential differentials and briefly discusses the associated symptoms and signs for many of them. Important differentials are almost always on the list. The discussions are usually too brief to be useful in themeselves- but I would generally go to a more comprehensive text for that sort of information. I use it as an aid-memoire to ensure I haven't missed an important differential in those rare cases.
