I think that there is a huge amount here which clearly needs to be said -I very much enjoyed reading your book on the future of the NHS. I have not seen a discussion on the matter that so comprehensively covers areas that many do not seem willing to broach.I thought your ideas were spot on- I think it is an excellent summary of both the problems and solutions within our current NHS-We have lost the relationships between junior doctors, consultants and nurses exactly in the way that you describe and this has led to alienated professionals trying to look after alienated patients in an alienated system-Turning to solutions, I entirely agree that "the money should flow from what arises out of the doctor/patient encounter and be seen to do so". This can be achieved through your model of "The details of how this is to be carried out and funded needing medical supervision".
